Cas 1
Dans le cadre d’une épistaxis postopératoire après turbinoplastie, une embolisation artérielle percutanée des territoires sphéno-palatins et du territoire facial droit par microbilles Embogold® 500–700 microns était réalisée chez un homme de 29 ans. Trois semaines plus tard, le patient était hospitalisé pour des douleurs maxillaires droites avec irradiation cervicale et trismus résistants aux morphiniques. Il n’y avait pas de syndrome inflammatoire biologique. La tomodensitométrie (TDM)
Discussion
Nous rapportons trois observations de granulomes cutanés à corps étrangers après migration tissulaire d’un matériel d’embolisation, dans les suites d’une procédure d’embolisation des artères faciales, sphénopalatines ou maxillaires internes.
Les microsphères sont des implants d’embolisation solides, synthétiques, sphériques. Dans nos trois descriptions, il s’agissait de microsphères Embogold® 500–700 μm, commercialisées par Biosphere Medical, biocompatibles, hydrophiles, et non résorbables [5].
Conclusion
Nous rapportons trois cas inédits de granulomes sous-cutanés survenus à distance d’actes d’embolisation artérielle de la sphère cervico-faciale par microbilles Embogold®. L’inflammation induite par les microbilles a entraîné une destruction de la paroi des vaisseaux embolisés, permettant la migration de billes au sein du tissu cellulaire sous-cutané avec une réaction tissulaire granulomateuse importante. La survenue simultanée des trois cas reste inexpliquée pour un produit fréquemment employé.

The five patients were diagnosed with IA and comorbidities (age, arterial hypertension, diabetes, hypothyroidism) and were put under colchicine treatment for IA crisis 3 weeks ago.
